\u201cUntuk Learning Objectives PBL minggu depan adalah nomor 1-5, HARUS LENGKAP ya\u201d<\/h2>\n\n\n\n

Seharusnya sudah tidak asing bagi kalian untuk mendengar kata-kata yang dihuruf kapitalkan semua. Ya, kata-kata \u201charus lengkap\u201d.<\/p>\n\n\n\n

Bagi kalian yang belum paham tentang learning objectives (LO), ini sama saja seperti \u201ctugas\u201d tapi bedanya LO ini merupakan guiding parts atau points sehingga pembelajaran di kedokteran bisa lebih terstruktur dan tidak lari kemana-mana.<\/p>\n\n\n\n

Allegedly.<\/em><\/p>\n\n\n\n

Namun, terkadang beberapa tutor (pembimbing diskusi saat problem based learning atau disebut juga dengan PBL) sangat sayang dengan anak didiknya sehingga mereka memberikan LO sebanyak-banyaknya.<\/p>\n\n\n\n

Seolah-olah itu merupakan tanda kasih sayang layaknya orang tua menyuruh anaknya belajar demi masa depannya yang lebih baik.<\/p>\n\n\n\n

Namun, yang kadang tidak disadari adalah LO PBL sudah distruktur sedemikian rupa sehingga kalimat tersebut cukup luas yang perlu dicari namun tidak meluber-luber. Intinya \u201cjust enough<\/em>\u201d.<\/p>\n\n\n\n

Nah, sayangnya, bagi kalian yang baru mempelajari topik baru, kalian tidak tahu batas \u201cjust enough\u201d ini. Malahan, LO PBL itu merupakan indikator seberapa \u201cbanyak\u201d kalian belajar.<\/p>\n\n\n\n

\u201cLO lu berapa bro?\u201d<\/p>\n\n\n\n

\u201cCuma 10 nih, lu?\u201d<\/p>\n\n\n\n

\u201cGila, enak banget lu bro, gua ada 23 coba!\u201d<\/p>\n\n\n\n

\u201cTapi gua 10 juga ada anaknya bro!\u201d<\/p>\n\n\n\n

Ya, tipikal pembicaraan PBL yang membandingkan jumlah angka LO.<\/p>\n\n\n\n[embedyt] https:\/\/www.youtube.com\/watch?v=bAi1EuMyafE[\/embedyt]\n\n\n\n

Sebagai tutor yang pernah dikontrak kurang lebih 5 bulan untuk mengajar PBL, gua akan memberi tahu kalian bagaimana cara mengetahui bahwa LO yang kalian cari untuk PBL sudah cukup lengkap atau belum.<\/p>\n\n\n\n

Ya, kuncinya adalah cukup lengkap. Bukan meluber-luber.<\/p>\n\n\n\n

Untuk ilustrasi, saya akan mengutip dua paragraph dari artikel NEJM yang berjudul Acute on Chronic Liver Failure oleh Arroyo V, Moreau R, dan Jalan R (2020).<\/a><\/p>\n\n\n\n

Paragraf pertama:<\/p>\n\n\n\n

\u201cThe Model for End-Stage Liver Disease with the addition of the serum sodium level (MELD-Na) score, in addition to scores based on the number of failing organs, provides accurate prognostication for individual patients with acute-on-chronic liver failure. The main principle of treatment is to diagnose and treat the precipitating event and then provide supportive therapy\u201d.<\/p>\n\n\n\n

Bandingkan dengan paragraf kedua:<\/p>\n\n\n\n

\u201cIn a study using the Asian Pacific criteria for acute-on-chronic liver failure, 76% of patients with cirrhosis and hepatitis B virus\u2013related acute-on-chronic liver failure had complications, including bacterial or fungal infection<\/p>\n\n\n\n

in 32% of patients, the hepatorenal syndrome in 15%, and gastrointestinal hemorrhage in 9%.16 The 28-day and 90-day transplantation-free mortality rates were 27.8% and 40.0%, respectively\u201d.<\/p>\n\n\n\n

Bukannya mengatakan bahwa paragraf B tidak penting, namun untuk masa pre-klinik, kalian belum ditekankan untuk menginterpretasikan sebuah jurnal di PBL. Terutama, apabila tutornya sendiri tidak begitu fasih dalam journal reading.

Sehingga, yang harus difokuskan untuk LO PBL adalah paragraf A yang berupa statement langsung mengenai faktor prognostik dan tatalaksana yang perlu kalian ketahui. Apabila kalian menemukan paragraf seperti B, biasanya mereka akan mencantumkan 4-5 paragraf seperti ini yang berisi penelitian-penelitian sebelumnya. Yang harus kalian lakukan adalah langsung saja baca paragaraf penutupnya. Karena, setelah kalian capek-capek berusaha memahami 10 menit mengenai studi-studi tersebut, biasanya mereka akan menyimpulkan \u201cStudies are inconclusive….\u201d atau \u201cThere needs to be more studies done on….\u201d<\/p>\n\n\n\n

Nah, selain itu ada juga beberapa LO yang sebenarnya ga perlu kalian tanggapi. Contohnya? Misalnya kalian sedang belajar blok ginjal dan setelah blok ginjal akan ada blok endokrinologi. Di saat mempelajari blok ginjal, pasti ada penyakit ginjal kronis (PGK) kan? Nah, salah satu penyebab tersering PGK adalah diabetes mellitus (DM). TAPI, bukan berarti lu pada harus menguasai materi satu minggu DM yang akan dibahas di blok endokrinologi di kidney juga!<\/p>\n\n\n\n

\u201cSemua itu ada saat dan waktunya\u201d \u2013 Professor Oak<\/p>\n\n\n\n

Lalu, bagaimana cara kita menentukan sejauh mana LO PBL itu lengkap buat kita?

Pertama, tanyakanlah apakah kalian akan memiliki kesempatan untuk mempelajari ini di blok-blok berikutnya? Bila tidak yakin, tanyakan kepada kakak kelas.<\/p>\n\n\n\n


Bila jawbannya iya, carilah secukupnya atau kaitkan saja dengan penyakit tersebut tapi tidak usah LENGKAP.<\/p>\n\n\n\n

Kemudian, kalian carilah sesuai dengan sejauh mana kalina ingin tahu mengenai ini.<\/p>\n\n\n\n


Contoh, demam berdarah dengue. Karena pasti akan kalian temukan kasus ini, maka might as well kalian tahu dosis loading RL bila pasien datang dengan dekompensasi.<\/p>\n\n\n\n

Tapi, kalau kasusnya adalah Duchenne Muscular Dystrophy, ya cukup tau lah presentasi dan Gower sign itu apa. Kalau kalian tertarik banget barulah tahu genetik dan varian dari DMD ada apa saja.<\/p>\n\n\n\n

Intinya, yang menentukan Learning Objectives PBL itu lengkap atau tidak adalah diri kalian sendiri. Yang akan memberikan input dan cross-check apakah ada yang terlewat adalah tugas satu kelompok kalian dalam berdiskusi.<\/p>\n","protected":false},"excerpt":{"rendered":"
